The Hospice Stories Project

Funded by The Saint Paul Foundation, F.R. Bigelow Foundation, UCare Foundation and a Live Well at Home Grant from Department of Human Services (DHS).


In 2014, Minnesota Network of Hospice & Palliative Care formed a community partnership with ECHO-TPT, SoLaHmo and West Side Community Health Services. Together the partnership is working to increase awareness about the values that impact the choices individuals and families make at the end of life and to introduce hospice care as an important option.

The goal of the project is to expand resources and information about end-of-life care to Somali, Latino and Hmong communities. We also aim to raise awareness among hospice organizations and care providers about the values, beliefs and traditions that are important to individuals from various cultures and open the discussion around what it means to approach this work with cultural humility.

Honoring Values in End-of-Life Care Videos

These videos were designed to educate individuals and families from multicultural communities on end-of-life care. Speakers from the Somali, Hmong, Spanish and English-speaking communities present perspectives on the values, traditions and experiences that shape choices individuals and their families make at the end of life. Hospice care is introduced as one end-of-life care option that may benefit individuals and families across many cultural traditions.

 

Radio Stories

These poignant radio stories reflect individuals and families’ intimate experiences with hospice care in the United States. Featuring the experiences of families from Hmong, Somali and Latino communities in Minnesota, these stories highlight some of the challenges that these families face at end of life and the role quality hospice care can play in offering individuals and their family peace and comfort in their final months.

Lucinda shares the story of her husband’s heart disease and their family’s experience with hospice care

Pa revisits the story of her father's struggle with cancer and their family's experience with hospice care
