We bring providers, business partners and individuals together to increase knowledge, access services and strengthen advocacy for people living with a serious illness or experiencing the end of life.

Providing Timely, Actionable State-Specific Resources
Families and individuals rely on MNHPC to inform them about their options and guide them to proven programs and trusted partners. We are skilled at helping people during a difficult point in their life, offering them choices so they can meet the needs of their family.
We work in collaboration with regional organizations and national leaders to provide a state-wide perspective on advance care planning, palliative care and hospice care. In addition, we offer providers opportunities to further their professional knowledge.
Our advocacy efforts focus on working with policymakers to create greater awareness and access to palliative and hospice care.
