A multidisciplinary presentation on palliative medicine in primary care with the goal to provide the audience with tools to be able to explain what palliative medicine is, what the difference is between palliative care and hospice care and to have some basic tools for managing symptoms at the end of life. The program also aims to provide information on how social workers, chaplains, pharmacists, nurses, nurse practitioners and physician assistants and physicians work as a team to provide holistic care for these patients living with complex medical illnesses.
