Presenter Lores Vlaminck, RN, BSN, MA, CHPN will lead this interactive four hour session designed to provide home and community based service providers with resources and tools to help older adults and their families effectively prepare for end-of-lfe and serious illness care.

This interactive session will:

  • Outline the trajectory of serious illness and declining health with tools to provide education and resources to clarify questions often raised by clients and families
  • Describe the difference between a health care directive and a POLST
  • Compare and contrast palliative care and hospice care, including funding, coverage of care and services, and models of delivery
  • Outline the end-of-life resources for eligible veterans

This program has been designed to provide 3.5 contact hours for health professionals. Participants will receive a Certificate of Attendance. 

This training is free, but space is limited, so pre-registration is required.
