Multicultural ResourcesMinnesota Network of Hospice & Palliative Care is committed to increasing education about and access to hospice and palliative care for all Minnesotans. MNHPC has partnered with numerous community organizations to create and distribute resources with the goal of raising awareness and answering questions about hospice, palliative care, and advance care planning within diverse cultural communities across the state. We also seek to raise awareness among health care organizations and care providers and encourage conversation about cultural values related to caring for individuals experiencing serious illness or the end of life. The Hospice Stories ProjectFunded by The Saint Paul Foundation, F.R. Bigelow Foundation, UCare Foundation and a Live Well at Home Grant from Department of Human Services (DHS).
The goal of the project is to expand resources and information about end-of-life care to Somali, Latino and Hmong communities. We also aim to raise awareness among hospice organizations and care providers about the values, beliefs and traditions that are important to individuals from various cultures and open the discussion around what it means to approach this work with cultural humility.
