Speaker: Dr. Theresa Reichert (she/her), DNP, MAN, BSN, RN, NPD-BC

Bio: Dr. Theresa Reichert is a Transcultural Equity and Belonging Nurse Program Manager at CentraCare, an innovative role she created to build understanding across cultures in health care and leadership, support the growing cultural diversity in Minnesota's communities, and equip the organization and its leaders to strengthen cultural awareness and responsiveness. Theresa brings her experience as a nurse in Minneapolis and rural MN, as well as her experiences as a nurse across the globe to today's conversations. She was recognized this spring by the MN Hospital Association with the Innovative Nurse Award for creating and leading the Transcultural Equity and Belonging nurse role.​

Title: Beyond Geography: Rural Serious Illness Care and Disparities in Minnesota

Description: Where someone lives should not determine the quality of care they receive during serious illness. This session explores the realities of serious illness care in rural Minnesota, including workforce limitations, geographic and financial barriers, caregiver burden, and gaps in access to palliative and hospice care. Through patient stories, practical examples, and lessons learned from rural practice, we will also explore opportunities to strengthen care through workforce development, telehealth, community partnerships, and better support for patients and caregivers across the continuum of serious illness.

Bio: ​Sara Revier, APRN, CNS, is an advanced practice provider with a clinical focus in palliative and serious illness care. She brings more than 15 years of experience caring for people with serious illness across inpatient, outpatient, and community settings. Her work has also included developing palliative care programs, supporting serious illness care initiatives across multiple healthcare systems, and providing education and mentorship to clinicians.Having grown up in rural Minnesota and spent much of her career caring for rural patients and families, Sara brings both personal and professional perspectives to the challenges of accessing high-quality serious illness care outside of urban centers. Her work is grounded in helping people live as well as possible throughout serious illness and ensuring that patients and families have access to the right care, at the right time, and in the right place.
