Purpose of the Care Navigation Program
The MNHPC Care Navigation Program is a free, public-facing service dedicated to assisting individuals, families, and community groups in gaining a comprehensive understanding of palliative care, advance care planning, and hospice care. Our program aims to provide education, guidance, and compassionate support to ensure that Minnesotans can make informed decisions about their healthcare.

1:1 Care Navigation Sessions
• Free individual or family sessions
• Offered via phone or Microsoft Teams
• Topics include palliative care, hospice care, advance care planning, and navigating care options

Monthly Virtual Group Education Sessions
• Open to the public
• Accessible topics related to serious illness, caregiving, and planning

Community & Group Presentations
• Available by request for community organizations, employers, faith groups, and nonprofits
• Virtual or in-person as capacity allows
